Waiver and Release of Liability
By signing this form, I acknowledge and agree to the following:
1. Assumption of Risk. I understand that participation in mixed martial arts (MMA) training and activities at Dukes MMA involves inherent risks, including but not limited to physical injury, disability, or death. I voluntarily assume all such risks.
2. Release of Liability. I hereby release, waive, discharge, and covenant not to sue Dukes MMA, its owners, instructors, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury that may be sustained by me during participation in any activities at Dukes MMA.
3. Indemnification. I agree to indemnify and hold harmless Dukes MMA and its representatives from any loss, liability, damage, or costs they may incur due to my participation in activities, whether caused by negligence or otherwise.
4. Medical Authorization. I authorize Dukes MMA staff to seek emergency medical treatment on my behalf if I am unable to do so myself. I accept full financial responsibility for any medical treatment received.
5. Rules and Conduct. I agree to follow all rules, regulations, and instructions provided by Dukes MMA staff and instructors at all times. I understand that failure to comply may result in removal from the facility.
6. Media Release. I grant Dukes MMA permission to use photographs or video recordings of me taken during activities for promotional or educational purposes.
7. Governing Law. This waiver shall be governed by and construed in accordance with applicable local laws. If any provision is found to be unenforceable, the remaining provisions shall remain in full force and effect.
I have read and fully understand this Waiver and Release of Liability. I am signing this agreement freely and voluntarily.
